
Why TRT Isn't a Set-It-and-Forget-It Treatment
A testosterone prescription is not a treatment plan. It is a starting point. What happens after the first script, the labs you never see, the numbers nobody rechecks, is where responsible testosterone replacement therapy either holds up or quietly falls apart.
Over the past several years, the market for testosterone therapy has exploded. A 2023 industry study identified more than 220 online clinics prescribing TRT, and the number of new platforms launching each year has continued to climb since. Access has genuinely improved. Men who once waited months for an endocrinology appointment can now get evaluated and prescribed in days. That part of the shift is good.
What has not kept pace, in a meaningful share of these platforms, is what happens after the prescription ships. A prescription without a monitoring plan is not incomplete because of paperwork. It is incomplete because testosterone is a controlled substance that changes multiple systems in your body at once, and several of those changes are only visible on a lab report, not in how you feel day to day.
Here is what a complete TRT protocol actually monitors, and why each piece matters.
Estradiol: the hormone nobody talks about until it's a problem
Testosterone converts to estradiol in the body through a process called aromatization, driven largely by an enzyme called aromatase, which is more active in fat tissue. Some of this conversion is expected and even necessary. Estradiol plays a real role in male physiology, supporting bone density, cognitive function, and healthy libido.
The issue is not that estradiol exists. It is that the ratio between testosterone and estradiol has to sit in a workable range, and that range is different for every man depending on body composition, dose, and delivery method.
When estradiol climbs too high relative to testosterone, men commonly report mood instability, water retention, and gynecomastia (enlarged breast tissue) risk over time. More frustrating for most patients is what we call the libido paradox: testosterone numbers look strong on paper, yet sex drive and function decline anyway, because excess estradiol is working against the very outcome the patient came in for.
Go the other direction, and the picture is just as problematic. Men on aggressive doses of an aromatase inhibitor, or those whose estradiol simply runs low without correction, often lose joint comfort, mood stability, and libido itself. Some degree of estradiol is not a side effect to eliminate. It is part of a functioning hormonal system.

Hematocrit: the cardiovascular risk that shows up on a CBC, not on a symptom checklist
Testosterone stimulates red blood cell production through its effect on erythropoietin and iron regulation. For a meaningful percentage of men, this pushes hematocrit, the proportion of blood volume made up of red blood cells, high enough to thicken the blood and raise the risk of clotting events, including deep vein thrombosis and stroke.
This is not a rare or theoretical concern. It is one of the most consistently documented effects of testosterone therapy across the clinical literature. A retrospective study following TRT patients over a median follow-up of nearly three years found that 57 percent crossed a hematocrit of 46 percent, 23 percent crossed 50 percent, and 5 percent reached 54 percent, the level most guidelines flag as a reason to pause or adjust treatment. In the TRAVERSE trial, the largest randomized controlled trial of testosterone therapy to date at over 5,200 men, even using a lower-risk gel formulation, a small percentage of participants still crossed the critical hematocrit threshold and had to discontinue.
The mechanism is well understood, the risk is well documented, and the fix, when caught early, is usually straightforward: a dose adjustment, a change in delivery method, or in some cases a therapeutic blood donation to bring the number back down. None of that is possible if nobody is checking a CBC on a regular schedule. This is precisely the kind of issue that feels invisible right up until it is not.
PSA: monitoring, not fear-based avoidance
For years, testosterone therapy carried an unfounded reputation for causing prostate cancer. Two decades of subsequent research have not supported that claim. What the evidence does support is more nuanced and, frankly, more manageable: testosterone can accelerate the growth of an existing, undetected prostate issue, which is why monitoring matters even though testosterone itself is not believed to initiate cancer.
Endocrine Society guidance recommends a PSA baseline along with a clinical prostate evaluation before starting therapy, followed by a recheck between three and twelve months after initiation. Real-world data backs up why this window matters. In one cohort of hypogonadal men treated with testosterone, roughly 31 percent experienced a PSA rise of more than 1.4 ng/mL above their own baseline, and 13 percent crossed the absolute threshold of 4.0 ng/mL, the level guidelines use to trigger a urology referral. These are not men who were harmed by testosterone. They are men whose monitoring plan caught a signal early enough to act on it.
A PSA check is a five-minute blood draw layered onto labs you are already having drawn. Skipping it does not save meaningful time. It removes a safety net that most primary care visits, on their own, do not reliably provide for men in this age range.
Free T and SHBG: treating the number that actually reflects what your body can use
Total testosterone is the figure most men fixate on when they get their labs back. In many cases, it is the wrong number to anchor on. Sex hormone binding globulin, SHBG, is a protein that binds a portion of circulating testosterone and makes it biologically unavailable. What is left over, free testosterone, is what your tissues can actually use.
Two men can post identical total testosterone numbers and have meaningfully different free testosterone, and therefore meaningfully different symptoms and results, purely because their SHBG levels differ. SHBG itself shifts with age, body composition, thyroid function, and the therapy itself, which means a single baseline reading does not hold steady over the course of treatment.
A protocol that dials in a dose based on total T alone, and never revisits free T and SHBG, is optimizing for the wrong variable. Bodies change over the course of a year on therapy. A dose that was correctly calibrated at the two-month mark is not guaranteed to still be correct at month eight, and the only way to know is to recheck.
What a complete monitoring cadence actually looks like
Responsible TRT follows a structure, not a one-time event:
- Baseline labs before the first dose: full hormone panel, CBC, metabolic panel, lipids, PSA
- Recheck at six to eight weeks after starting, to confirm the dose is landing where it should
- Ongoing reassessment on a regular cadence after that, adjusting for estradiol, hematocrit, PSA, free T, and SHBG as the picture evolves
- A physician reviewing every result, not an algorithm flagging out-of-range values with no clinical context attached
This is not bureaucracy layered onto a simple treatment. It is the treatment. A prescription without this structure is not a lesser version of TRT. It is an incomplete one.
