What Actually Happens When You Walk In

July 06, 20265 min read

Most men who consider Springhouse never make it to the first appointment. Not because they decided against it, but because the unknown got in the way first.

What's the room like. What do they ask. Do I need to take my shirt off. Is this going to feel like a doctor's office or something else entirely. Is anyone going to know why I'm there.

None of that is dramatic on its own. But stacked together, it's enough friction to keep a $100 consult sitting unbooked for months, sometimes longer. This is the entire first visit, laid out in order, with nothing left out.

BEFORE YOU ARRIVE: DIGITAL INTAKE

The process starts before you ever walk through the door. You complete intake digitally: health history, current medications, what brought you in, and a short questionnaire on symptoms and goals. This happens on your own time, not on a clipboard in a waiting room.

Start privately, at your own pace.

Dr. Lee-Agawa reviews everything before your appointment. That changes what the first conversation looks like. Instead of spending the first ten minutes of a twenty-minute consult re-collecting information you already provided, the visit starts at the part that actually requires a physician in the room.

This is not a small operational detail. A 2021 study published in the Journal of Medical Internet Research examined the effect of pre-visit digital intake on consultation quality and found that when administrative and historical information was collected beforehand, physicians spent measurably more time on substantive clinical discussion during the visit itself. The intake form is not paperwork. It is what makes the rest of the visit efficient.

THE SPACE: WHERE YOU ACTUALLY GO

Springhouse operates inside Grace and Glow MD's clinical space in Ambler, Pennsylvania. There is one shared entrance, and a separate door specifically for Springhouse once you're inside.

Your first visit starts before you arrive, with a private digital intake reviewed by Dr. Lee-Agawa ahead of your consultation.

This is a deliberate design choice, not an afterthought. The goal is not to build a separate building or a hidden identity. The goal is that nothing about walking in signals to anyone, staff or other patients, what kind of appointment you're there for. Discretion here is functional, not theatrical. It is built into the layout, not promised as a marketing line.

THE CONSULTATION: WHAT ACTUALLY GETS DISCUSSED

You sit down with Dr. Lee-Agawa. The conversation starts from what you already submitted, not from zero. She asks targeted follow-up questions based on your specific intake, not a generic script.

From there, she walks through what bloodwork would tell her that a conversation alone cannot, and why that distinction matters clinically. This is a structured clinical conversation, not a sales conversation, and the structure exists for a specific reason.

The Endocrine Society's Clinical Practice Guideline on testosterone therapy states plainly that a diagnosis of low testosterone requires confirmed low levels on two separate morning blood measurements in conjunction with consistent symptoms. A single conversation, however thorough, does not meet that bar, and Dr. Lee-Agawa does not treat it as if it does. Nobody leaves this consult with a diagnosis based on how they describe feeling that day. The bloodwork is the diagnostic backbone. The consult is where that gets explained, contextualized, and ordered.

THE BLOODWORK: WHAT GETS ORDERED AND WHY IT'S MORE THAN THE STANDARD PANEL

If bloodwork is indicated, it gets ordered the same day, no separate appointment required.

The Springhouse Men's Panel includes: testosterone (free and total) with SHBG; estradiol via LCMS; FSH and LH; prolactin; PSA total; a complete blood count; comprehensive metabolic panel; lipid panel; TSH; and hemoglobin A1C.

A discreet clinical entry point designed to make the first appointment feel clear, private, and low-pressure from the moment you walk in.

This is meaningfully broader than what most men get from a standard primary care testosterone check, which frequently measures total testosterone alone and stops there. A 2016 review published in the Journal of Clinical Endocrinology & Metabolism examined exactly this gap and found that total testosterone in isolation, without SHBG and a calculated free testosterone value, can produce both false negatives and false positives depending on an individual's sex hormone binding globulin levels. Two men with identical total testosterone numbers can have very different clinical pictures once SHBG is factored in. The panel exists specifically to close that gap.

After the draw, labs are processed and results return through Decoda Health, the practice's electronic medical record system, typically within a few business days.

AFTER THE VISIT: WHAT HAPPENS TO YOUR RESULTS

Dr. Lee-Agawa reviews your results personally and follows up with you directly. This is not a portal notification with a number and no context. It's a conversation about what the results actually mean and what, if anything, makes sense as a next step.

There is no built-in pressure point here. If your bloodwork comes back unremarkable, that's genuinely useful information, and it gets communicated as such. If something points toward an actionable next step, you'll hear exactly what that looks like and the clinical reasoning behind it.

WHAT THIS ACTUALLY REMOVES

The barrier standing between most men and their first appointment was never the bloodwork itself, or the conversation, or even the cost. It was simply not knowing what either one would actually involve.

Now you know. Digital intake on your own time. A consultation built around your specific history, not a script. A panel broader than what you'd get elsewhere, ordered the same day. Results delivered with context, not just numbers.

The first appointment is a $100 consult, applied toward any services that follow. It starts with a conversation. Nothing more is decided until you've had it.
